  1. OPEN THE FORM

Use the link below to be taken to the Docusign Powerform Signer.  Enter your full name and email address, then scroll down and select “Begin Signing” (yellow box towards the bottom). Review the disclosure and agree to use electronic signature. Then select “Continue” (yellow box).

  1. REVIEW AND COMPLETE THE FORM

Select “Start” (yellow box) to be taken to the first field on the form for completion.  Press the tab button (on your keyboard) to be taken through the fields.  You can also use your mouse.  Fields outlined in red are required, fields outlined in gray are optional.

  1. COMPLETE AND SUBMIT THE FORM

Select “Finish” (yellow box at the bottom of the last page of the form) once the form is complete.  NOTE: If you forgot to fill out a required field, you will be prompted to complete it, and not be able to finish the form. 

Once finished, your browser will be automatically directed to the SCU careers page, a copy of the completed form will be automatically emailed to you, and to Human Resources for processing.
